29 CFR 1910.212(a)(3)(ii):  Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a. On or about October 24, 2012, in the detail area the Controlled Automation's rebuilt Whitney angle line shear had a programmable controled operation to punch and cut angle steel without a point of operation guard, exposing the operator to an amputation hazard.     b. On or about October 24, 2012, in the detail area the Peddinghaus model: Pedimax 110/140 universal steelworker with notching and punch machine was operated without a point of operation guard at the punch and stripper position, exposing operators to an amputation hazard.    c. On or about October 24, 2012, in the detail area the Pacific plate shear model: 800G12 was operated without the point of operation from the backside of the shear being guarded, exposing employees to an amputation hazard.
