29 CFR 1926.62(d)(2)(iv):     Until the employer performed an employee exposure assessment as required by 29 CFR 1926.62(d), and documented that the employee(s) performing any of the tasks listed in 29 CFR 1926.62(d)(iv) were not exposed to lead in excess of 2,500 micrograms per cubic meter, 50 times the Permissible Exposure Limit, the employer did not treat the employee(s) as if the employee(s) were exposed to lead in excess of 2,500 micrograms per meter cubed, and did not implement employee protective measures as prescribed in 29 CFR 1926.62(d)(2)(v):    Jobsite:    Employees welding and torch cutting lead coating were not treated as if their lead exposure exceeded 2,500 micrograms per cubic meter.  For example, employees did not use respiratory protection or other appropriate personal protective equipment.

29 CFR 1926.502(d)(16)(iii):     Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    Jobsite:    Connectors used six foot lanyards along with 'beamers' to provide fall protection.  This exposed them to potential free falls in excess of six feet.

29 CFR 1926.753(d)(2)(i):     Materials being hoisted shall be rigged to prevent unintentional displacement;    Jobsite:    Metal decking bundles was rigged using two slings in a basket configuration.  This exposed connectors receiving the bundles and persons on the ground to a stuck-by hazard from the improperly secured bundles.

29 CFR 1926.754(b)(3):     A fully planked or decked floor or nets was not maintained within two stories or 30 feet (9.1 m), whichever is less, directly under any erection work being performed.    Jobsite:    The ground and first floors were only minimally decked.  This exposed connectors working on the roof to potential falls into the basement of approximately 34 feet.
