Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a. On or about October 25, 2012, on a 4:12 pitch roof, employees attaching roofing tiles to the roof were not protected from a 10-foot fall hazard by the use of a fall protection system.
