29 CFR 1910.23(a)(9):     Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through.    a)  The platform between Kiln #1 and #2, on or about October 26, 2012, the floor opening between the  platform and side walls of  the kiln were not guarded.  There were openings on both sides of the platform ranging from 9 inches to13 inches wide between the Kiln side walls and the platform, exposing employees to the hazard of stepping into floor openings.            ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.132(a):     Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a)  The platform between Kiln #1 and #2, on or about and at times prior to October 26, 2012, an employee walked on top of a 4-inch wide steel beam that supported the monorail sling arm without fall protection, exposing the employee to 7 and 24-foot fall hazards.                   ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.254(d)(9)(iii):     Cables with damaged insulation or exposed bare conductors shall be replaced. Joining lengths of work and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall have insulation adequate for the service conditions.   [55 FR 13709, Apr. 11, 1990; 61 FR 9227, March 7, 1996; 70 FR 53929, Sept. 13, 2005]    a)  Maintenance shop  on or about and at times prior to October 25, 2012, employees were stick welding metal frames with damaged welding lead, exposing employees to electrical hazards.    b)  Maintenance shop  on or about and at times prior to October 25, 2012, the welding cable connected to the negative weld output terminal of the Lincoln electric welder was in need of repair due to exposed copper wiring and damaged insulation, exposing employees to electrical hazards.    ABATEMENT CERTIFICATION IS NOT REQUIRED
