29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a) Jobsite - 806 Summit Drive, McKeesport, PA: On or about October 23, 2012, three employees were observed without head protection while cleaning up debris on the ground, two employees were throwing shingles off of the roof to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) Jobsite - 806 Summit Drive, McKeesport, PA: On or about October 23, 2012, an employee was observed disconnected from his body belt while descending a residential roof measuring approximately 21 feet from ground to eve with a 10/12 pitch.

(a) Jobsite-  806 Summit Drive, McKeesport, PA: On or about October 23, 2012, two employees were observed working on a residential roof measuring approximately 21 feet from ground to eve, with a 10/12 pitch, wearing a body belt.
