29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: On or about October 24, 2012, the employer did not ensure that electrical equipment used by the employees was free of the hazards likely to cause death or serious physical harm to employees. Employees were exposed to shock and/or electrocution. 1. A 100-foot yellow electrical extension cord being used by employees had severe insulation damage in several places. 2. A Makita Circular Saw being used by an employee at the time of inspection had cable damage, the cable's outer insulation was damaged as well as one of the conductors inside, The inside conductor was cut bare and exposed. 3. A Porter Cable Electric Reciprocating Saw used by employees had outer insulation cord damage in the strain relief area.

Employees were exposed to a fall hazard of approximately 6 feet to the ground.
