29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  On or about October 31, 2012, employees were laying brick on the exterior wall of the residence from a work platform located on the top tier of the scaffold which was not fully planked, exposing employees to the hazard of an 18 foot fall.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each platform on all working levels of scaffolds are fully planked.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section.  On or about October 31, 2012, employees were laying brick on the front elevation of a residence from the top tier work platform which was not provided with a guard rail system, exposing employees to the hazard of a fall of 18 feet.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees on scaffolds more than 10 feet above a lower level are protected from falling to that lower level by fall protection.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  On or about October 31, 2012, employees were using the cross braces and frames for access and egress to and from the different levels of the scaffold where a ladder not provided for access, exposing employees to the hazard of a fall of up to 18 feet in height.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that ladders are provided to employees accessing scaffolding.
