29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:    On or about 10/31/2012, at the NE corner of SR 7 and Lake Worth Rd, an employee was observed working from an aerial lift without being attached to the basket.  The employee was exposed to a fall hazard of approximately 20 feet.
