29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Hulick Metals, Inc., did not establish a written lockout program, nor were machines locked out when machines were being serviced or maintained. Servicing and maintenance is performed on saws.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.333(a):     Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized:    Hulick Metals did not employ safety-related work practices when the operator of the #9 Wells Saw opened the live, 220 volt, electric panel in order to push a reset button.    Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1910.333(c)(10):     An interlock system was not returned to its operable condition when the work was completed:    On the 220 volt electric panel for the #9 Wells Saw, the interlocked guard was not returned to its operable condition after the reset button was pushed, exposing the operator to live parts.     Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1910.335(a)(1)(i):     Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    An employee opened a live electric panel, for the #9 Wells Saw, in order to push the reset button, and was not provided with nor wore appropriate personal protective equipment, including, but not limited to:  cotton clothing, hearing protection and leather gloves.    Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).
