29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Etch Room, Chemi-Graphic, 340 State Street, Ludlow, MA; Employees entering or working in the Etch Room were not protected from exposure to hydrofluoric and nitric acid by personal protective equipment exposing the employees to chemical burns. Instance 1: October 22, 2012 - employee working in the etch room was using an air pump to transfer nitric acid out of a 55 gallon barrel when a hose clamp failed spraying the employee with nitric acid causing burns. The employee was wearing safety glasses, a fabric ball cap, apron, and gloves. The personal protective equipment needed for this operation includes: face shield, full chemical suit, gloves, and boots. Instance 2: October 25, 2012 - during the walkthrough inspection the employee leading the CSHO through the etch room was wearing reading glasses in lieu of the posted requirement for safety glasses and the Operations Manager entered the room with no eye protection at all while an employee was spraying chemicals off of the metal plates with water toward the entrance and personnel aisle where the team was walking and into the work space.
