29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification.    On or about October 31, 2012, employees who used and attended the shower facility for the asbestos containment located at Terminal B, Row B-102, were exposed to an electric shock hazard.  A Power Sentry Model 100201 relocatable power tap that was labeled, "for indoor use only" was taped to the side of the containment beneath the plumbing fixtures.

29 CFR 1926.1101(j)(1)(i)(B): Shower facilities that comply with 29 CFR 1910.141(d)(3) were not provided or demonstrated to be not feasible.    On or about October 31, 2012, employees who worked in an asbestos containment, established as a regulated area and located at Terminal B, Row B-102, were exposed to asbestos.  The shower was not functional.
