29 CFR 1910.67(c)(2)(v):     A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:    a) Marathon Gas Station - an employee working from a Wilkie Manufacturing ladder truck was exposed to an approximate 20 foot fall hazard, in that, the employee was not provided with a restraint device, observed on or about 11/2/2012.     b) Marathon Gas Station - an employee working from a Telsta truck mounted aerial lift was exposed to an approximate 22 foot fall hazard, in that, the employee was not provided with a restraint device, observed on or about 11/2/2012.
