29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a On or about November 2, 2012, an employee cutting framing pieces with a 5007MG Makita handsaw did not have safety glasses, exposing himself to being hit by flying debris. b. On or about November 2, 2012, an employee performing framing operations was using a Hitachi NR83A2 pneumatic air nail gun without safety glasses, exposing himself to being hit by nails or flying debris.

On or about November 2, 2012, an employee performing framing operations was not protected from a 11 foot fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternate fall protection.
