29 CFR 1910.95(g)(1):     The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:    a) Exclusive Services & Pallet Co., Inc. - An employee operating the pop up saw was exposed to noise levels of 92.8 dBA, exceeding the action level of 85 dBA. Audiometric testing had not been conducted, on or about 10/30/12.    b) Exclusive Services & Pallet Co., Inc. - An employee operating the denailer was exposed to noise levels of 98.4 dBA, exceeding the action level of 85 dBA. Audiometric testing had not been conducted, on or about 10/30/12.    c) Exclusive Services & Pallet Co., Inc. - An employee repairing wood pallets was exposed to noise levels of 97.1 dBA, exceeding the action level of 85 dBA. Audiometric testing had not been conducted, on or about 10/30/12.    d) Exclusive Services & Pallet Co., Inc. - An employee repairing wood pallets was exposed to noise levels of 94.3 dBA, exceeding the action level of 85 dBA. Audiometric testing had not been conducted, on or about 10/30/12.    e) Exclusive Services & Pallet Co., Inc. - An employee repairing wood pallets was exposed to noise levels of 93.6 dBA, exceeding the action level of 85 dBA. Audiometric testing had not been conducted, on or about 10/30/12.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
