29 CFR 1926.451(g)(1):   Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a)  581 Hartford Tnpk., Killingly, CT. - On or about 11-2-12 - Employees were observed working on a Hydro Mobile scaffold platform system 16 feet above the ground with standard railing systems missing exposing employees to fall hazards.    b)  581 Hartford Tnpk., Killingly, CT. - On or about 11-2-12 - Employee was observed working on a frabricated metal frame scaffold installing concrete block 12 feet above the ground with no standard railing installed on the end of the work platform exposing him to a fall hazard.

29 CFR 1926.701(b):     All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:    a)  581 Hartford Tpke, Killingly, CT. - On or about 11-2-12 - Employee was observed working on the end of the Hydro Mobile scaffold system platform 14 feet above unprotected vertical rebar.    b)  581 Hartford Tpke., Killingly, CT. - On or about 12-2-12 - Employee was observed on a fabricated metal scaffold system working 12 feet above unprotected vertical rebar.
