29 CFR 1926.25(a):     During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.          (a) Construction Site - On or about October 30, 2012 employees were exposed to fall and nail puncture hazards from falling on wood boards with nail protruding from them. Lumber with protruding nails were observed in four different areas of the construction site.

29 CFR 1926.404(b)(1)(iii)(C):     Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.          (a) Construction Site - On or about October 30, 2012 the employer did not remove or replace damaged electrical cords and electrical equipment daily prior to use.  Employee was exposed to an electrical shock hazard by using a Skil circular saw plugged into an open outlet box supplied electrical power from a defective power cord. The electrical cord supplying electricity to the outlet box had two different cords spliced together to form one power cord.

This outlet box with no cover was laying on the ground where live electrical parts were exposed to moisture.
