A.)  Wagners Energy Center, Inc., 3924 Peach Street, Erie, PA 16509, roof of the structure, on or about November 5, 2012 and times prior thereto:  Employees were performing roofing work 16.75 feet above ground level on a flat roof without a means or method of fall protection.
