29 CFR 1910.132(d)(2)  The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    On October 16, 2012, at the Huff Salt Water Disposal #32-11 site, the employer did not certify, in writing, that a hazard assessment of the workplace was done to determine whether hazards were present which necessitate the use of personal protective equipment.  Two employees were working inside of pit #2, power washing the lining from debris and contaminants while wearing personal protective equipment (ppe). The employer had not made a determination that the ppe would be sufficient to protect employees.
