On or about November 5, 2012 at the maintenance office and upstair storage area:    The top wood handrail on the stairway used by the employees to access the storage area above the maintenance office was cracked, loose and not properly installed to protect the employees from a 7' 10" fall hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points:    On or about November 5, 2012 at the oven five production area of the facility;    Employees operating and working around the thermoforming machine five (oven five) were not protected from the 41"W X 8'L unguarded opening at the forming press area of the machine to prevent the employees from being crushed.
