(a)  Champion Construction Inc., 3831 Utica, Denver, CO:   On or about 11/5/2012, the controlling employer had not ensured excavated dirt was not placed directly next to the east side edge of the 8 foot 6 inch deep basement excavation.  Condition exposed employees of a sub-contractor to stuck by hazards associated with falling/rolling dirt and/or rocks into the excavation while installing concrete foundation forms.    (b)  On and prior to 11/5/2012, the controlling employer had not ensured residue hardened concrete material debris were removed from the south and southwest edge of the 10 foot deep excavation. Condition exposed employees of a sub-contractor to struck by hazards associated with concrete debris falling into the excavation while installing concrete foundation forms.

(a)  Champion Construction, 3831 Utica, Denver, CO:  On or about 11/5/2012, the controlling employer in charge of the worksite did not ensure the east side of the 8 foot - 6 inch deep basement excavation was appropriately sloped, benched, equipped with a support/shield system, or designed by a registered professional engineer (RPE) to prevent cave-in. Condition exposed the sub-contractors employees installing wood foundation forms to "struck by" hazards associated with cave-in.      (b) On or about 11/5/2012, the controlling employer in charge of the worksite did not ensure the southwest corner of the 10 foot deep basement excavation was appropriately sloped, benched, equipped with a support/shield system, or designed by a registered professional engineer (RPE) to prevent cave-in. Condition exposed the sub-contractors employees installing wood foundation forms to "struck by" hazards associated with cave-in.
