29 CFR 1910.304(g)(6)(VI)(C)(1)    Exposed noncurrent carrying parts of cord- and plug- connected equipment that may become energized shall be grounded under the following condition:  If the equipment is of the following type: Refrigerator, freezer and air conditioner.      An AC outlet located in the employee break room near the refrigerator has an open ground.   The outlet has the refrigerator plugged into it.

29 CFR 1910.305(b)(1)(i):     Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:    In the employee break room near the refrigerator, the AC outlet had unprotected wires/conductors entering the box without the proper use of an entrance gland.  The wires were exposed and the outlet was powering the refrigerator.
