29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    a)  Western Reserve Roofing & Home Improvement, Ltd. on or about July 28, 2012, at the address of 31643 Compass Cove Avon Lake, OH: The employer failed to ensure employees were protected from falls while removing old and installing new roofing material on a residential type structure.  Employees were exposed to an approximately 18 ft. fall to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    a)  Western Reserve Roofing & Home Improvement, Ltd. on or about July 28, 2012, at the address of 31643 Compass Cove Avon Lake, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems, guardrails or alternative measures while performing roofing operations atop a residential type structure.
