Conshohocken Dental Office for Advanced Smiles: On or about November 5, 2012 - The employer did not offer the Hepatitus B vaccination or obtain the documentaion required under section 1910.1030(f)(2)(iv) for employees working as dental hygienists/technicians and who were exposed to blood-borne pathogens within 10 working days of their initial assignment.  NOTE:   Vaccination is not required for those employees who had previously received the complete hepatitis B vaccination series; antibody testing has revealed that the employee is immune; or the vaccine is contraidicated for medical reasons.

A copy of the regulation  B. A description of the exposed employee's duties as they relate to the exposure incident  C. Documentation of the route(s) of exposure and circumstances under which exposure occurred  D. Results of the source individual's blood testing  E. All medical records relevant to the appropriate treatment of the employee including vaccination status which are the employer's responsibility to maintain.
