29 CFR 1910.147(c)(8):     Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance:    a) On or about November 8, 2012, the employer did not assure that each authorized employee placed a lockout device on the energy disconnects on the printing machines prior to performing maintenance operations.  One authorized employee applied their locks and tags on the electrical disconnect while other employees assisted with the work.

29 CFR 1910.1200(f)(10):     The employer did not ensure that workplace labels or other forms of warnings were legible    a)  On or about November 8, 2012, the employer did not ensure the labels on the cans containing Type Wash were legible.  The labels on the cans were covered in paint and were unable to be read.
