29 CFR 1926.452(c)(2): All brace connections were not secured: On or about 11/07/2012, employees were exposed fall, struck by, or caught in or between hazards while working on the North West side of an apartment building located right behind the Carbon Construction trailer. The first level of the scaffold cross brace had a broken pin which was then secured to the scaffold frame with electrical wire. There were five levels of scaffold above this point.

29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question: On or about 11/07/2012, employees were exposed fall, struck by, or caught in or between hazards while working on an apartment building located right behind the Carbon Construction trailer. The first level of the scaffold cross brace had a broken pin which was then secured to the scaffold frame with electrical wire. There were five levels of scaffold above this point. The employer did not have a competent person inspect the scaffold prior to use.
