Caring First, Inc. dba Breese Nursing Home, Breese, IL: On or about November 9, 2012, the employer did not correctly classify the following work related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2010:  a) On or about 5/25/10, a recordable injury occurred when an employee hurt their entire right side after tipping over a mat.  The days of on-job transfer or restriction were incorrect and were adjusted to 66 days after consulting the injured employee's file. b) On or about 11/24/10, a recordable injury occurred when an employee turned and fell on their knee in a resident's room.  The days of on-job transfer or restriction were incorrect and were adjusted to 16 days after consulting the injured employee's file.

29 CFR 1904.7(b)(3): How do I record a work-related injury or illness that results in days away from work? When an injury or illness involves one or more days away from work, you must record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known.  Caring First, Inc., dba Breese Nursing Home:  On or about November 9, 2012, the employer did not correctly classify the following workplace injuries on the OSHA 300 log for the calendar year 2012 [column K, (days away)]:  a) Case number 4, employee slipped in urine and hurt knee, resulting in 7 days away, but was recorded with restricted days only. b) Case number 7, employee was transferring resident and twisted knee, resulting in 33 days away, recorded with an incorrect number of days away.

Caring First, Inc. dba Breese Nursing Home:  On or about November 9, 2012, the employer did not provide the required injury descriptions on the OSHA 300 log for the calendar year 2012:   a) On or about3/19/2012, a recordable injury occurred when an employee tripped over a cord and fell.  The description in Column F did not describe which knee and how it was injured.  b) On or about 5/25/2012, a recordable injury occurred when an employee slipped in water and fell.  The description in Column F did not describe the body part injured or how it was injured.  c) On or about 5/26/2012, a recordable injury occurred when an employee hurt their back when rolling a resident.  The description in Column F did not describe which portion of the back was injured or how it was injured.  d) On or about 6/28/2012, a recordable injury occurred when an employee slipped in urine and hurt their knee.  The description in Column F did not describe which knee was injured or how it was injured.  e) On or about 8/4/2012, a recordable injury occurred when an employee pulled their back lifting a resident.  The description in Column F did not describe which portion of the back was injured or how it was injured.  f) On or about 9/1/2012, a recordable injury occurred when an employee slipped on water on the floor.  The description in Column F did not describe what body part was injured or how it was injured.  g) On or about 10/12/2012, a recordable injury occurred when an employee twisted their knee transferring a resident.  The description in Column F did not describe which knee, or provide a description of the transfer, i.e. from where to where.
