29 CFR 1926.1053(b)(1):   Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:         This violation most recently occurred on November 09 2012, at a workplace located at 6221 Woodcrest Lane, Dallas, TX 75214; where at least five (5) employees used an extension ladder to access the roof structure, and the ladder's side rails did not extend at least 3 feet above the upper landing surface being accessed.
