29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used.    a)  Residential dwelling, northwest corner of Ellsworth and South Aiken Avenues, Pittsburgh, Pennsylvania, 15232.  Observed on or about November 07, 2012, two employees were working from a ladder jack scaffold platform that was not equipped with a means of access.  The employees were exposed to a fall hazard of approximately 15 feet to the ground below.

29 CFR 1926.451(g)(1)(i):  Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a)  Residential dwelling, northwest corner of Ellsworth and South Aiken Avenues, Pittsburgh, Pennsylvania, 15232.  Observed on or about November 07, 2012, two employees using a ladder jack scaffold were not protected from falling by the use of a personal fall arrest system.  The employees were exposed to a fall hazard of approximately 15 feet to the ground below.
