29 CFR 1910.332(b)(1):     Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:    Dons Automotive Group, LLC, located at 720 N. Shoop Ave., Wauseon, OH 43567: On or about November 13, 2012, the employer did not ensure that a maintenance employee was provided with training on electrical safety-related work practices such as, but not limited to, using test equipment to verify that the circuit elements and equipment parts of the fluorescent light fixtures in the facility were deenergized prior to the replacement of electrical ballasts, in order to protect the employee from electrical hazards. The maintenance employee replaced electrical ballasts of the fluorescent light fixtures without conducting proper verification that circuit elements and equipment components were deenergized, and was exposed to electrical hazards.    No additional abatement information is needed for this item.

29 CFR 1910.333(b)(2)(iv)(B):     Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that:    Dons Automotive Group, LLC, located at 720 N. Shoop Ave., Wauseon, OH 43567: On or about November 13, 2012, the employer failed to ensure that a qualified person used test equipment to test the circuit elements and electrical parts of the fluorescent light fixtures in the facility in order to ensure that the circuit elements and equipment parts were deenergized. The maintenance employee replaced electrical ballasts of the fluorescent light fixtures without conducting proper verification that circuit elements and equipment components were deenergized, and was exposed to electrical hazards.    No additional abatement information is needed for this item.
