Shoop Ave., Wauseon, OH 43567: On or about November 13, 2012, the employer did not ensure that a maintenance employee was provided with training on electrical safety-related work practices such as, but not limited to, using test equipment to verify that the circuit elements and equipment parts of the fluorescent light fixtures in the facility were deenergized prior to the replacement of electrical ballasts, in order to protect the employee from electrical hazards. The maintenance employee replaced electrical ballasts of the fluorescent light fixtures without conducting proper verification that circuit elements and equipment components were deenergized, and was exposed to electrical hazards.
