29 CFR 1926.502(d)(15): Anchorages used for attachment for personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee:    (a)Sondrup Roofing & Sheet Metal @ 1805 S. Foothills Highway, Boulder, CO 80303: On and before October 7th, 2012, the exposing contractor did not ensure that anchorages used in conjunction with rope grab systems were adequate and would support an employee in the event of a fall. The reusable roof anchors installed on the parapet wall did not have an adequate amount of screws/nails that the manufacturer requires for use, thus reducing the effectiveness during a fall event and not meeting the 5000 pound anchorage requirement for personal fall arrest systems. The condition exposed an employee to a fall hazard of approximately 14 feet.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. The condition exposed employees to the hazard of falling from an elevation of approximately 14'.
