OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn hazards: a)Dolco Packaging Corporation - The employer did not ensure that the pipes at the extruder line were covered or isolated to prevent employees from contacting parts with surface temperatures of 240 degrees F to 250 degrees F while working and walking adjacent to the extruder area on or about 11/07/12. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter: (a) Employees throughout the facility were exposed to fire hazards. Employees performing duties when working with butane for processing foam cartons are exposed to static electricity, and printing employees are exposed to ultra violet lights for drying ink. Production employees and/or fire brigade employees required to use fire extinguishers had not been trained on an annual basis on or about 11/07/12. The last fire extinguisher training documented was on July 26, 2011.
