29 CFR 1910.27(d)(2)(ii)  All landing platforms were not equipped with standard railings and toeboards, so arranged as to give safe access to the ladder. Platforms were less than 24 inches in width and 30 inches in length.    Produce Dock Area, for employee(s) who ascended/descended a wooden fixed ladder to a 11 1/4" W X 46" L landing platform (plank) to retrieve product/shelving from the storage loft, the open platform was not guarded.  The fall hazard measured approximately 11' 9".        OR IN THE ALTERNATIVE:    29 CFR 1910.23(c)(1) Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard:    Produce Dock Area, for employee(s) who ascended/descended a wooden fixed ladder to a 11 1/4" W X 46" L landing platform (plank) to retrieve product/shelving from the storage loft, the open platform was not guarded.  The fall hazard measured approximately 11' 9".

29 CFR 1910.27(f)  Ladders were not maintained in a safe condition.  Ladders were not inspected regularly, with the intervals between inspection being determined by use and exposure:    Produce Dock area, the fixed wooden ladder used by employee(s) to retrieve fruit baskets, shelving racks, etc. from the overhead storage loft was not maintained in a safe condition as evidenced by the following:      a)  Bottom rung was missing.      b)  Wood rails were cracked in several places.  Wood was missing at the base of the right side of the ladder.      c)  Nails were pulling away from the board where the ladder was attached.      OR IN THE ALTERNATIVE:    29 CFR 1910.27(b)(1)(ii)  The distance between rungs, cleats, and steps shall not exceed 12 inches and shall be uniform throughout the length of the ladder:    Produce Dock area, the fixed wooden ladder used by employee(s) to retrieve fruit baskets, shelving racks, etc, from the overhead storage loft was missing the bottom rung.

29 CFR 1910.305(a)(2)(x)  Flexible cords and cables were not protected from accidental damage, as might be caused for example, by sharp corners, projections, and doorways or other pinch points:  Deli department, the flexible cords belonging to the 3 tier portable display cooler ran along the floor under the narrow swinging door to the deli counter.  These flexible cord(s) were not protected from accidental damage from the employee(s) who traveled through this swinging door daily and also created a tripping hazard.
