On or about Wednesday November 14, 2012 in the open areas of the facility;    Employees are exposed to a fall hazard ranging from 9 feet up to 37 feet to ground level from the 30 X 16 and 26 X 16 floor openings with midrails not properly installed to protect the employees from a fall hazard.      The instances floor openings with missing midrails were located on the following levels:    a). Third floor 30 X 16 and 26 X 16 openings were missing midrails.    b). Second floor 30 X 16 and 26 X 16 openings were missing midrails.    c). First floor 30 X 16 and 26 X 16 openings were missing midrails.
