29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about November 6, 2012, the employer failed to ensure employees working from a 5/12 pitch roof were protected from a fall of approximately 11 feet 2 inches by using an adequate fall protective system/devices. Employees were putting new roofing/weatherproofing in place and were not fully protected.
