29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:        On or about November 16, 2012, employees engaged in sewer line repair activities utilized an extension cord to power electrical de-watering pumps and a General Model EP8 ventilator. The power cord was not provided with strain relief exposing employees to electrical shock hazards.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:        On or about November 16, 2012, the employer did not ensure electric cords that employees were using were not worn or frayed exposing employees to electrical shock hazards.     a) Employees engaged in sewer line repair activities utilized an extension cord that was damaged in multiple locations exposing inner electrical wiring to power electrical de-watering pumps and a General Model EP8 ventilator exposing the employees to electrical hazards.     b) Employees engaged in sewer line repair activities utilized General Model EP8 ventilator with a cord that was damaged exposing inner electrical wiring exposing the employees to electrical hazards.

On or about November 16, 2012,at a work site located at Hwy 121 & 6900 Freeway, Richland Hills, Texas; at least two employees working in a 8-foot 4-inch deep trench that was not shielded in accordance with manufacturers specifications, exposing the employees to hazards of a cave-in.
