29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  On or about November 8, 2012, a 480 VAC connection did not have strain relief devices in place.  The fitting that connected to the terminal was missing and the fitting that held the cord to the plug was tight around the outer sheath of the cord.  The connection was located just outside the upper dog house and energized sub structure pumps.  This condition created an electrical hazard to rig workers.
