29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:     On November 14, 2012, at a jobsite located at 1604 East Brook Drive, McKinney, Texas; at the front of the building, on a 4x2 tier tubular welded frame scaffold, employees climbing cross-braces and end frames were exposed to a fall hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    On November 14, 2012, at a jobsite located at 1604 East Brook Drive, McKinney, Texas; at the front of the building, employees working from a 4x2 tubular welded frame scaffold while doing masonry work were exposed to a fall hazard. The scaffold was not provided with guardrails and the employees did not wear personal fall protection.
