29 CFR 1910.107(b)(5)(i):     The spraying operations except electrostatic spraying operations shall be so designed, installed and maintained that the average air velocity over the open face of the booth (or booth cross section during spraying operations) shall be not less than 100 linear feet per minute. Electrostatic spraying operations may be conducted with an air velocity over the open face of the booth of not less than 60 linear feet per minute, or more, depending on the volume of the finishing material being applied and its flammability and explosion characteristics. Visible gauges or audible alarm or pressure activated devices shall be installed to indicate or insure that the required air velocity is maintained. Filter rolls shall be inspected to insure proper replacement of filter media.    (a) Location - Spray booths:    On or about 11/16/12, the emlpoyer failed to ensure that spray booths were properly arranged in the following ways:     1. Air velocity at the face of the booth(s) appeared to be less than 100 linear feet per minute  2. Visible gauges were either broken or missing and could not indicate if the required air velocity was maintained    3. Filters had not been inspected as some pre-filters were missing from the exhaust system

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    (a) Location - Production:    On or about 11/16/12, the employer failed to provide machine guarding of a Thermwood 5-axis CNC Trimming machine.     (b) Location - Production:    On or about 11/16/12, the employer failed to provide machine guarding of a Fadal Vertical Machining Center.      (c) Location - Production:    On or about 11/16/12, the employer failed to ensure table guarding at the rear of a Thermwood 3-axis CNC Trimming machine.
