29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person: On or about November 15, 2012, an employee was exposed to approximately 20-foot fall hazards when the employee was working from a job-made scaffold on building 1519. The scaffold consisted of a board bridging the gap between the end of the pump jack scaffold and the saw horse located on the top apartment deck and decking extending from the sawhorse to the apartment decks floor. A design by a qualified person would not have included deficiencies such as, but not limited to, the lack of fall protection and pump jack poles that were not braced at the bottom.
