29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:     a)  At the site, on November 15, 2012, where an employee was installing sheet metal siding on a newly constructed building from a JLG aerial lift, model #660SJ, serial #1258110300051649, without being connected to the basket.  This condition exposed the employees to a fall hazard of greater than 28 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  At the site, on November 15, 2012, where an employee was standing on a roofing structure that was greater than 28 feet in height while installing sheet metal siding on a newly constructed building without the use of fall protection.
