(b) On or about 3/7/13 - at the No. 2 Nitric Acid Plant, documentation was not available for PSV-003 and -004 on the Coolant Chiller for Frick (9-223-C2). (c) On or about 3/7/13 - at the No. 2 Nitric Acid Plant, documentation was not available for PSV-005 and -006 on the Coolant Chiller for Frick (9-223-F1). (d) On or about 3/7/13 - at the No. 2 Nitric Acid Plant, documentation was not available for PSV-007 and -008 on the Frick Compressor (9-223-J).

29 CFR 1910.147(d)(1): The authorized employee did not have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy before the authorized or affected employee turned off the machine or equipment:  (a) On or about 11/13/12 - at Urea reactor 1303-D, the isolation package 1303-D (Urea Reactor) procedure #42IP006 dated 1/31/12, that was used in conjunction with a completed line breaking permit, did not inform employees of the magnitude of energy being controlled.
