29 CFR 1926.150(a)(2): Access to all available firefighting equipment was not maintained at all times: On or about November 7, 2012 the fire extinguisher provided was secured with plastic tie wraps making it difficult to access and necessitating the use of a tool to cut the straps. This exposed workers to a fire hazard at the construction site.
