29 CFR 1910.303(b)(1)(i): Safety of equipment was not determined using considerations for the suitability of installation and use in conformity with the provisions of this subdivision: On or about November 8, 2012, household type relocatable power strips were daisy chained to provided extra outlets in the upper dog house. The daisy chaining of power strips created a fire hazard to workers.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: On or about November 8, 2011, the following temporary electrical equipment was being used as fixed wiring. a) A portable fan was tied the electrical conduit on the ceiling of the SCR trailer. It was energized to a SJTW orange extension cord tied the the trailer and plugged into a 110 VAC outlet in a box that is approved only for inside walls of residential construction. These conditions created an electrical hazard to workers. b) A surge protector was attached to the wall of the upper dog house to provide extra outlets. It was plugged into a surge protector and used to energize the cordless phone used for communication to the company trailer.

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and cables were attached to building surfaces: On or about November 8, 2012 the following 110 VAC flexible cords were attached creating an electrical hazard to employees: 1. 110 VAC surge protector was fixed to the wall, located in the upper dog house. This temporary outlet was used to energize alarms and other temporary power strips. 2. 110 VAC SJTW orange extension cord was used to energize a temporary fan chain to electrical conduit on the ceiling of the SCR trailer.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: On or about Novermber 08, 2012 the metal condit on the roof of the mud house had a break. The conduit fed the 110 VAC outlets in the mud house all of which were not grounded. This condition exposed workers in the mud house to an electrical hazard.

The employer was previously cited for a violation of this Occupational Safety and Standard or which was contained in OSHA inspection number: 312238710, Citation Number: 1, Item Number: 18b and was affirmed as a Final Order on Feburary 12, 2012, with respect to a workplace located at Rig 107, Pyote, TX 79777:

29 CFR 1910.157(e)(1): The employer did not provide for the inspection, maintenance and testing of all portable fire extinguishers in the workplace: On or about November 8, 2012, The following fire extinguishers were missing their inspection seals creating a fire hazard to oil rig workers. A "third party" was responable for inspecting the fire extinguishers: 1. BADGER, Z-964757 20A 120 BC fire extinguisher located at mud pits. The seal was missing. 2. BADGER, CO2 Model B10V, Z-7329118, 10LB type BC fire extinguisher, located electrical trailer. The seal was missing. 3. ANUSL Sentry af-108489 20A 120BC, located in motor room. The seal was missing. 4. Large fire extinguisher located at the accumulators. The seal was miissing and the label was not legible.
