29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:      28 Bowling Green Pkwy, Jefferson, NJ.    Employee was not trained to recognize hazards associated with scaffolds. Employee was working out of an aerial lift basket without the use of his harness/body belt to keep him from being thrown out of the aerial lift basket.                                          Violation observed on or about 10/26/2012.
