29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards: a) 744 Ave E, Bayonne, NJ. Employee observed working off a pump jack scaffold was not provided training regarding scaffolds. Hazards observed which included, but were not limited to safe access and egress, thereby exposing employee to fall hazard. Violation observed on or about 11/19/2012.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used: a) 744 Ave E, Bayonne, NJ. An employee was observed egressing from pump jack scaffold onto portable ladder that was approximately 2 feet away from the scaffold, thereby exposing employee to a fall hazard. Violation observed on or about 11/19/2012.
