On or about November 2, 2012: (a) While performing cleaning operations employees were exposed to struck by hazards associated with cleaning nozzles being used not having effective chip guarding tips installed, air nozzles were producing more than 70 p.s.i out of nozzle tip.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use. On or about November 2, 2012: (a) Employees engaged in cleaning procedures were exposed to respiratory hazards when the employer required use of respirators but failed to implement and maintain a respiratory protection program to include training, fit test and medical evaluations.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. On or about Novemebr 2, 2012: (a) Employees were exposed to burns and smoke inhalation hazards when a fire extinguisher located within the facility was blocked by materials awaiting further processing making fire extinguisher inaccessible to employees.
