29 CFR 1910.147(c)(4)(ii):     The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy:      A.)  BWP Hardwoods - On or about 11/20/12, employees performed maintenance and servicing on equipment including, but not limited to the head saw.  The lockout/tagout procedures for the  machinery did not include, the steps to verify deenergization, instructions on how to drain stored energy, notification of affected employees, steps for the release of lockout, energy sources, and magnitudes of energy.  The maintenance and servicing activities included but were not limited to, oiling machinery, blade changes, and repairs of machinery.

29 CFR 1910.147(c)(6)(i):     The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:        A.)  BWP Hardwoods - On or about 11/20/12, employees were utillizing lockout/tagout on machinery including, but not limited to, a headsaw.  The employer had not conducted a periodic inspection of the lockout/tagout procedures to check for any deficiencies.  The maintenance and servicing activities included but were not limited to, oiling machinery, blade changes, and repairs of machinery.
