Location: 87 Bowne Street, Brooklyn, NY. Outside Aisle storage area.         LP power industrial truck fuel gas cylinder was not supported against being knocked over. On or about 10/26/2012.            NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

Location:     87 Bowne Street, Brooklyn, NY.     a) Operator was using a Yale power industrial truck without functionaland operating lights. On or about 10/26/2012    b) A caterpillar 906 powered industrial truck's windshield and driver side mirror were broken. On or about 10/26/2012 .        NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY of 1,000.00, IN ACCORDANCE WITH 29 CFR 1903.19.
