29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    (a) Rear Roof of the CITGO Quik Mart located at 152 Riverside Parkway, Austell, GA 30168: Employee was exposed to shock hazards while installing roof shingles in close proximity to the energized power feed into the building. The employee was working within a foot of the energized power circuits as they were not guarded or de-energized prior to the start of work.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    (a) Front side of CITGO Quik Mart building located at 152 Riverside Parkway, Austell, GA 30168: Employees were exposed to fall hazards while using a TranzSporter (Model LH2000) Platform Hoist Ladder to access the roof located at heights of approximately 13'7" from the ground. The manufacturer of the ladder hoist states that the ladder should not be used as a personnel ladder.
