29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     WORKSITE: The employees walking and working on the following roofs without adequate fall protection system, exposing them to falls between 10-9 and 18-4 to the ground.

29 CFR 1926.552(a)(1): The employer shall comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators. Where manufacturer's specifications are not available, the limitations assigned to the equipment shall be based on the determinations of a professional engineer competent in the field.    WORKSITE: The employees were accessing the back side of the roof using the materials hoist in lieu of a ladder exposing the employees to falls up to 18'-4".

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.    WORKSITE: The side rails of the bottom half of a Werner model D30028, 28 foot aluminum extension ladder, used by employees to access the roof did not extend at least three (3) feet above the roof eave and the ladder was not secured to prevent accidental displacement.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).    WORKSITE: The bottom half of a Werner model D30028, 28 foot aluminum extension ladder, used by employees to access the roof was setup at an angle where the horizontal distance from the top support to the foot of the ladder exceeded one-quarter(1/4) of the working length of the ladder.
